Required Documentation

To qualify for the $2.00 off Limited cable and $2.00 off Signature cable discount, the following criteria must be met:

• Applicant must be 65 years of age or older with proof of age
• Applicant must be the account holder
• Applicant must be eligible to receive one of the following: (must send proof)
  • Supplemental Security Income
  • Medicaid Benefits
  • Veterans Service Benefits

Please complete this form and return to Astound. Requests may take up to 14 business days in order to be processed. The request will be denied if the required documentation does not accompany this form. Submitting false documentation or misrepresenting your identity may result in legal action in which case you may be penalized to the fullest extent of the law.
